Required items for all children enrolled in Kaleidoscope Kids
- Kaleidoscope Pickup form
(DUE JUNE 15)
- Allergy form
(IF APPLICABLE - DUE MAY 20)
- Authorization to administer medication
(IF APPLICABLE - DUE MAY 20)
Note: Separate form required for each medication
If your child is enrolled in one of the "camp" courses that Kaleidoscope offers, you must submit the below items. These courses are: 36, 63, 83, 91, 103, 115, and 131. If your child IS NOT in one of these courses, please DO NOT submit these additional forms.
- Completed Health History form
(DUE MAY 20)
Copy of child's immunization records (DUE MAY 20)
Copy of child's most recent physical exam (DUE MAY 20)
Note:Physical exam must have taken place no more than 18 months before the start date of course.
